The Pen Is Hiding in Plain Sight
The first object is probably the easiest once you know where to focus.
Look carefully at the doctor.
More specifically, examine the doctor’s coat.
The pen is tucked inside the coat pocket.
It’s positioned somewhere completely natural for a pen, which is precisely why your eyes may initially ignore it.
Your brain sees a doctor wearing a coat and treats everything attached to that coat as part of the overall scene.

Three to go.
Now Look Behind the Nurse
The second object requires you to stop focusing on the people.
Examine the wall behind the nurse.
There you’ll find a small red apple.
Because it’s incorporated into the background rather than placed somewhere you’d naturally expect to find fruit, it can take a little longer to notice.
Once you see the red shape, however, it becomes much easier to recognize.
That’s two.
The Egg Is Much More Cleverly Disguised
The third hidden object isn’t sitting on a table.
It isn’t being held by either person.
And it isn’t somewhere in the background.
Instead, the puzzle uses an object’s shape to hide it.
Look closely at the nurse’s collar.
The way the collar is formed creates the recognizable outline of an egg.
That’s why many viewers overlook it.
They aren’t searching for an egg-shaped pattern.
They’re searching for a literal egg.
And that difference is exactly what the puzzle uses against you.
